Twice? I only counted once, and it wouldnt have been hard to fake.
Look at how many times Jason or Freddy have been ressurected. They'll milk this until its bone dry...so far so good, but it'll start going downhill pretty damn soon.

I don't think the 4th will "bring Jigsaw back". We don't know the fate of the doctor and photographer (Jigsaw said he cleaned up Amanda's mess) from the first movie and dead or alive Jigsaw has another game already in progress.

Would have been quite hard to fake his heartbeat stopping too. /SPOILER. I know what you're saying, but I cant see them "bringing him back to life" in the way say, Jason would be brought back. I cant see him being in a sequel, only a prequel, in flashbacks or just not at all. :(
