so are any of the sequels worth getting?

well the sequel/prequel (whichever you want to call it) is enjoyable

which would you say is better the original 3
or three extremes?

I read somewhere (unfortunately, I can't recall the source) that Miike-san stated that he was tired of making horror movies, and didn't really want to do any more. I think that this really came across in "Box." I liked the film, but it seemed... workmanlike. That's the best word I can think of to describe it. "Dumplings" was the best of the three, in my opinion. I'm going to have to seek out the other films in the series, now that I know about them.

three has 1 good story
three extremes has 2

hmmm...
maybe another sequel will have three good stories :p
