Hmmm... Depends on what you're looking for - I'm definitely not much of an exploitation fan, so I stay away from those films (just not my taste). What are you looking for in terms of "nasty/extreme"? It can mean so many things in this genre, you know? What are some movies that you've watched that you'd like to see more of?
If you're into torture/exploitation, check out
Mum & Dad. It's the most recent one that I've seen. Again, not a huge fan of the genre, but if that's what you like, then you should check it out.
Dread was pretty tortured. Little indie film that made me feel icky.
Couple of "extreme" films that have gotten a lot of buzz around the forum but I haven't checked out yet are
Martyrs (I think this one is definitely torture/exploitation from what I hear) and
Antichrist (this one is supposed to be twisted and arty at the same time - It's currently on Netflix Instant and I will probably check it out soon).

À l'intérieur (Inside) - Pretty extreme, violent slasher/home invasion flick. Great stuff.
Grace might be up your ally, too - Pretty twisted - A lot of people liked it around here (though personally I thought that it could push the envelop more).
Takashi Miike is known as being an "extreme" Asian director (he's the one that directed Visitor Q that I think you should watch). He's also done the following, which are definitely disturbing and worth a watch if this is what you're looking for:
- Audition
- Ichi the Killer
- Gozu (people are torn with this one, though - I thought it was brilliant, but a lot of people I talk to HATED it)
SPEAKING of Asian exploitation, this is a favorite of Doc's - A lot of people around here like it (though I wasn't a fan as I mentioned, not my genre):

Ferox and Elvis Christ can probably recommend stuff you'd like - They seem to like exploitation films. Hopefully they'll chime in. :)
